What is the referral process?
- Referrals can be made by fax: 403-538-6433
- Forms are available above OR you cansend us a referral letter.
- Referrals will be accepted from: physicians, nurse practitioners, physiotherapists (with prac ID)
- We will confirm the receipt of the referral within 2 weeks.
- Patient will be notified directly of their appointment.
Other Services
Other services including Physiotherapy, Counselling, Social Work, Health Care Navigation do not require a referral. Information can be found on this website or contact us for details at 403-300-2476
